Please note: We do not accept exchange plans purchased through the Colorado Marketplace (Connect for Health Colorado).
To ensure you receive the maximum benefit from your insurance and avoid any unexpected costs, we strongly recommend that you verify your in-network status and coverage details directly with your insurance provider prior to your appointment. Your insurance carrier can confirm whether we are in-network with your specific plan and provide accurate information regarding copays, deductibles, coinsurance, and any referral requirements.
Accepted insurance plans are subject to change. If you have questions about whether your plan is accepted or need help verifying your benefits, please contact our billing office at 719-867-7800 (Option 6).
Referral
A number of insurance plans require a referral or authorization to be seen by a specialist. As we are unable to determine the specific requirements for all insurance plans, please work with your primary care physician and insurance company to complete the referral or authorization process.
UnitedHealthcare or HMO
If you have UnitedHealthcare or any other HMO (Health Maintenance Organization) insurance plan, a referral from your Primary Care Physician (PCP) is required before we can schedule your appointment with our specialists.
Here’s exactly how the process works:
- Contact your PCP first: Call your PCP’s office and let them know you need to see a specialist (please mention our practice name and the reason for the visit).
- Your PCP obtains the authorization: Your PCP will review your needs and submit the referral/authorization request directly to UnitedHealthcare or your HMO plan. Important: Our office cannot request or obtain this authorization for you—it must come directly from your PCP.
- Once approved, you can schedule with us: After your PCP’s office receives approval from the insurance plan, they will let you know. You can then call our office to book your appointment.
Please note: Without this PCP-issued authorization on file, we will not be able to see you for your visit. This is a requirement of your insurance plan, not our practice.

Insurance Card(s)
Please bring your current insurance card(s), primary and secondary (if applicable). We will scan your card(s) into our practice management system to ensure we have the correct information to file claims with your insurance company.
Co-Pay
If your insurance plan requires a co-pay for a physician’s office visit, we will collect the applicable amount when you check in. Keep in mind that our physicians are specialists, and your insurance plan may, thus, require a higher co-pay than for a primary care physician office visit.
Cash Payments
We accept cash payments only in the exact amount due. We appreciate your understanding and encourage you to prepare the precise amount.
FMLA Paperwork
Please note that we charge $25 for completing FMLA paperwork.
